About this notice

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our responsibilities

  • We are required by law to maintain the privacy and security of your protected health information (PHI).
  • We will let you know promptly if a breach occurs that may have compromised your PHI.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you authorize us in writing.

How we may use and disclose your PHI

For treatment

We may use and share your PHI with your providers, other providers, laboratories, and pharmacies as needed to coordinate your care.

For payment

We may use and share your PHI to bill and get paid from insurance plans or other payers, for example, to confirm your coverage before a session.

For health care operations

We may use and share your PHI to run our practice, improve your care, and contact you when necessary, such as for quality assessments or credentialing activities.

Other uses that do not require your authorization

  • Public health and safety activities required or permitted by law
  • Research, subject to strict privacy protections and IRB oversight
  • Compliance with the law, court orders, or subpoenas
  • Responding to organ and tissue donation requests
  • Working with a medical examiner or funeral director
  • Addressing workers' compensation, law enforcement, and other government requests

Your rights

  • Access: Get an electronic or paper copy of your record.
  • Amend: Ask us to correct information you think is incorrect or incomplete.
  • Confidential communication: Request that we contact you a certain way (e.g., phone vs. email) or send mail to a different address.
  • Restrictions: Ask us not to use or share certain PHI for treatment, payment, or operations. We are not required to agree but will try to accommodate.
  • Accounting of disclosures: Get a list of the times we've shared your PHI outside of treatment, payment, and operations (up to 6 years prior).
  • Complaints: File a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice

We may change this notice at any time. Changes apply to all information we have about you. New notices will be posted in-app and on our website and mailed upon request.
